Our clinic and hospital-based facility are equipped with a state-of-the-art surgical facility designed for patient comfort, convenience, privacy, and safety. The environment is highly professional and we value quality medical and surgical care.
Clinic Hours
Our clinic hours are from 9:00 AM to 5:00 PM Monday through Friday, with Wednesday hours sometimes variable. Emergency care for our patients is available 24 hours a day. Your time is important to us. Since there are several steps involved in an initial evaluation, possibly including photographs, please plan on spending several hours with us during your initial visit.
Insurance and Payment Policy
Most insurance companies often cover procedures that are considered reconstructive in nature, in part or in full. We will be happy to submit your insurance claim for you.
Procedures that are considered cosmetic in nature, are elective, and therefore are not covered by insurance. We will be happy to assist you in making appropriate inquiries regarding insurance coverage. Fees for elective cosmetic surgery are discussed fully with you so that payment can be made in advance of your surgery. Our financial advisor is available to meet your needs and help you identify alternative financial arrangements.

Patient Forms
The following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time, you will save a significant amount of time during your visit. Please be prepared to provide this information to our office along with your current medical insurance information. If you have any questions when filling out these forms, please do the best you can and our staff will assist you with your questions on the day of your visit.

Cancellations
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. This courtesy, on your part, will make it possible to give your appointment to another patient.
